Community Aged Care ProgramThe Community Aged Care Program provides Community Aged Care Packages (CACPs) for frail aged people with complex care needs who live in Stonnington, Glen Eira or Port Phillip local government areas (LGAs).
What is a Community Aged Care Package (CACP)?
A Community Aged Care Package (CACP) is a flexibly tailored package of care designed to assist frail older people with complex needs to live at home as safely and as independently as possible.
Who is eligible for a CACP?
CACPs are targeted at frail older people (over 70 years or over 50 years for Aboriginal and Torres Strait Islander people) who would be assessed as requiring at least low level residential aged care, have complex care needs and have expressed a preference to live at home.
To be eligible, a person must be assessed by the Aged Care Assessment Service (ACAS) under the guidelines above. Once assessed as eligible the application will be placed on a waiting list until a place becomes available with Prahran Mission.
Referral to ACAS can be directly through self referral, or through a health or community service, local GP. Contact information on the local ACAS can be made through Commonwealth Carelink Centre 1800 052 222
What does Prahran Mission provide as part of a CACP?
Prahran Mission provides experienced case managers who can assist the client to develop an individual care plan which is monitored and regularly reviewed to ensure that any changing care needs are taken into account. Services which can be organised as part of a package of care can include:
What is the cost?
A fee is applicable for administering the package and will be based on the person's capacity to pay taking into account living expenses, and any exceptional and unavoidable expenses.
The amount charged forms part of an agreement Prahran Mission and the care recipient and is based on guidelines provided by the Commonwealth government.
The agreement also outlines rights and responsibilities of both parties, details of the type of care and complaints procedure.
