Inner South Outreach Program

 

What is Inner South Outreach Program?

 

It is a Psychiatric Disability Support Program funded by the Mental Health Branch; Department of Human Services. It is a home-based outreach service, which provides psychosocial rehabilitation for people aged 16 - 64, recovering from a mental illness.

 

 

What the program aims to do?

 

The focus of the program is to build on existing skills and attempt to promote activities that encourage sustainable and meaningful participation within the community.

 

 

What we do?

 

The program is a supportive home-based outreach service which means that our work is conducted in the community, either at your home or an agreed location. When you join the Program you will be allocated a key support worker who will work with you throughout your involvement with the program. You and your support worker will generally meet once per week.

 

The period of time you are involved in the Program varies depending on the identified needs and goals.

 

 

What sort of support does the Inner South Outreach Program Offer?

 

The program supports the process of recovery through the development of therapeutic relationships and working collaboratively with you to identify, develop and implement an Individual Recovery Plan.

 

The Individual Recovery Program plan will articulate your goals, skills and strengths, and provide you and the support worker direction and purpose.

 

For example, this may take the form of working with you towards the self management of your own general well-being, establishing your own safe and comfortable physical environment, developing meaningful activities and facilitating access to a range of community service providers such as mental and general health professionals, housing services, education, employment, vocational, counselling, social and recreational agencies.

 

 

Who can access the program?

 

People aged 16 to 64 who are recovering from mental illness and reside in the Cities of Stonnington, Port Phillip and Glen Eira.

 

 

How to access the program?

 

The referral process is usually initiated by telephone. Referrals can be made to the program by health professionals, self referral or family / carer.

 

The program is a free service. However some of the activities you choose to participate in within another agency may incur a cost. Involvement in the program is voluntary.
