Role Of Second Story Key Worker

 

Each person that joins Second Story is allocated a key worker.  It is this persons role to:

  • Assist you to identify rehabilitation goals and develop chosen skills that will be of benefit to you in the broader community, both immediately and in the future when you leave the program.  They will be supportive and offer guidance, advice and assistance but it is important to remember they are not counsellors.  If you need ongoing counselling they will advise you to seek professional help.

Phases of Program.  The key workers role has a different emphasis depending on what stage you are at in the program.  There are generally three phases when using the Second Story program, and this is how your key worker will assist you to move through each stage:

 

Joining Phase

To meet and make you feel welcome

To explain what the service can and cannot offer

To be friendly, supportive and trusting

To identify and acknowledge your strengths

To discuss possibilities within the program

To explain the concepts of Recovery and Rehabilitation

Participating Phase

Establish a user friendly goal plan that will assist you to succeed in achieving your dreams.

To help develop a recovery plan that is an effective working tool for you.

To help you to identify which groups will help you achieve your goals and refer you on to other services where appropriate.

Where desired and/or necessary, contact and coordinate meetings with your clinical or other supports.

Encouragement to look beyond the program and think about using the service to prepare for life after rehabilitation.

To support you in taking a break from the program when necessary.

 

Moving On Phase

Helping you to identify and prepare for what is next.

Help you to find and access mainstream services.

Review your journey through Second Story.

To remain supportive and reassure you that moving on from Second Story does not mean you can’t remain in contact, or re-engage with the program if the need arises in the future.

     

Your key worker appreciates that moving on can be scary and a hard thing to do.  They will work with you on making this transition as easy as possible, and rest assured you will not be abruptly removed from Second Story (unless there is a serious code of conduct issue).  Our whole philosophy is based on working with you to achieve your potential in a safe, supportive and non-threatening way.  Your key worker will guide you through the three phases of Second Story and listen to your concerns while at the same time keeping you on track to graduate successfully from rehabilitation into the broader community.
