Home Based Outreach provides comprehensive, home based services to a range of consumers in and across the Southern and Eastern Metropolitan regions. Our programs provide assistance to such groups as the frail aged, women with a psychiatric illness (with dependent children in their care) and people living in Kinkora and Wynnstay hostels. We support people to live independently and safely in a way that is meaningful to them. Participation in all our programs is voluntary.

Our services are coordinated by experienced case managers. We aim to connect people with networks within their local community and we work collaboratively with a range of other service providers to ensure that consumers are adequately supported and their presenting needs met in a timely and appropriate manner.

All consumers of this service have a structured, individualised program that documents their goals, strategies, support required and reviews the outcome of this work.
